{"id":6542,"date":"2025-07-14T11:09:08","date_gmt":"2025-07-14T11:09:08","guid":{"rendered":"https:\/\/www.prisonersadvice.org.uk\/?p=6542"},"modified":"2025-07-14T11:09:08","modified_gmt":"2025-07-14T11:09:08","slug":"prisons-and-probation-annual-report-shows-35-increase-in-investigations-into-deaths","status":"publish","type":"post","link":"https:\/\/prisonersadvice.org.uk\/wordpress\/2025\/07\/14\/prisons-and-probation-annual-report-shows-35-increase-in-investigations-into-deaths\/","title":{"rendered":"PRISONS AND PROBATION ANNUAL REPORT SHOWS 35% INCREASE IN INVESTIGATIONS INTO DEATHS"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">The 2024-25 Prisons and Probation Ombudsman (PPO) Annual Report has now been published, detailing prison and probation complaints &#8211; as well as&nbsp;investigations into fatal incidents.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Tragically, the PPO started investigations into&nbsp;486&nbsp;deaths last year &#8211; a 35% increase on the year before. Of these, 398 deaths occurred in prisons &#8211; 106 more than last year. 73 deaths occurred within two weeks of release from prison &#8211; 20 more than the previous year.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Throughout the year, 506 recommendations were made following deaths in custody, of which:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>165 were made about healthcare provision<\/li>\n\n\n\n<li>62 about general administration<\/li>\n\n\n\n<li>62 about suicide and self-harm prevention<\/li>\n\n\n\n<li>37 about emergency response<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">5,100 prison-related complaints were received over the course of the year &#8211; up by 755 on the previous twelve months.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Common categories of complaints were:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Property (868)<\/li>\n\n\n\n<li>Staff behaviour (224)<\/li>\n\n\n\n<li>Administration (139)<\/li>\n\n\n\n<li>Adjudication (135)<\/li>\n\n\n\n<li>Categorisation (107)<\/li>\n\n\n\n<li>Incentives &amp;
